Nurse Assessment & Care Planning
Clients requiring ongoing advocacy, care coordination, transition planning, or membership support may begin with a comprehensive session.
We review medical history, current concerns, medications, providers, support systems, upcoming procedures, and goals to identify potential gaps and create a personalized advocacy and support plan.
A full assessment may not be required for limited, one-time services. PointRN will explain the recommended starting service and complete cost before paid work begins.

PointRN works with healthcare providers, medical practices, hospitals, senior-living organizations, employers, benefit professionals, insurers, and community organizations. Scope, deliverables, timeline, fees, and payment terms are negotiated under an individual contract.
Important Information
Every patient’s situation is different. PointRN will explain the recommended level of support, what is included, and the anticipated cost before services begin. Services are private-pay and generally not covered by health insurance.
PointRN provides advocacy, consulting, coordination, and educational support. PointRN does not diagnose medical conditions, prescribe medications, make medical decisions, or replace licensed medical providers. We work alongside the patient’s existing healthcare team.
